Every year, up to 210,000 Australians are admitted to hospital due to medication problems.

At least 50 per cent of these admissions could be avoided by better medicine management. Now you can make an appointment with your pharmacist to help you understand your medicines and stay out of hospital.

MedsCheck is a service funded by the Australian Government and involves one-on-one time with your pharmacist, in the pharmacy to discuss all your medicines. This service is free.

A MedsCheck allows you to work together with your pharmacist to ensure you are getting the most out of your medicines.

The service basically helps you better understand your medications and give you confidence you are taking them at the right time and in the right quantities. Also, if you have diabetes, or know someone who does, there is now also a new service to manage your diabetes with the help of your pharmacist.

This session is called a Diabetes MedsCheck. The purpose of the Diabetes Meds Check is to help you manage your diabetes so you can your reduce your risk of developing complications. The session lasts up to 45 minutes and you can discuss with your pharmacist all aspects of your diabetes management.

Firstly, your pharmacist will review your medications and make sure you are getting the most benefit from of them.

From this, you’ll understand how best to take your medications so you don’t get sick. As everyone is different, you can discuss how to manage your diabetes within your lifestyle.

Your pharmacist will help you develop a plan that covers the important things you need to do each day and set goals to aim for. You’ll also get any information you need on your medications, blood glucose testing or anything else about your diabetes.
